CoGP - QSE Affidavit Template

If you are an entity that doesn't fall into a specific sector with it's own B-BBEE code, your annual turnover is between R10 million and R50 million AND you have black ownership of 51% or more, you are entitled to merely complete an Affidavit as evidence of your B-BBEE status.
